Get Started in 3 Easy Steps

Call Biosports to make an appointment. Depending on your needs we will be able to make an appointment for you within the next couple of days. If you have experienced an acute injury, we will do everything we can to accommodate you. Often times it is important in your recovery to reduce swelling and start therapy right away. We will ask what we will be seeing you for, and how recent your injury was, this will help determine your need for treatment.

Once we have you on our schedule, we will ask you for your insurance information. It is very helpful to have this at the time the appointment is made. If we have the insurance information we can call to verify benefits. We want to make sure you are fully informed of what your benefits are and what your financial responsibility will be. Insurance can be confusing, and often times benefits are different than what the patient thought they were.

After an appointment has been made, if time allows we will send you a packet with patient information. This includes a registration sheet, a patient health history sheet, a subjective evaluation form (symptoms described by the patient), and directions to the office. By filling this out ahead of time you will avoid having to fill it out in the office and will be ready for the therapist when they are ready for you. If we don’t have time to send it in the mail, we ask that you come 20 minutes before your appointment to fill it out. These forms are also available here on the web site.

Arrive at Biosports a little early for your first appointment. If you have the paperwork filled out you only need to come 10 minutes before your appointment. If you will be doing the paperwork at Biosports before your appointment you will need to arrive at least 20 minutes early. We will need a little time to go over your insurance benefits, and get your chart ready for the therapist. You will need to wear clothing that allows access to the injured area. Your first visit will be a thorough evaluation will include a movement analysis. Our therapists not only evaluate but treat on the first visit.

We provide tank tops and shorts for you to wear during you visit if you choose. Many find this convenient if they have forgotten their own clothes or need to come from work.
